Liability Waiver

I understand that in participating in the Transform8 programme I am required to exercise which will include strength, flexibility and aerobic exercise, including the use of equipment, meaning I will be physically exerted and that may in turn, be strenuous and has the potential to cause me possible physical injury or harm and even death. I am fully informed of these risks and any complications due to exercise and aware that it is my responsibility to consult with a physician prior to starting the Transform8 programme. In reading and signing this waiver I am hereby agreeing and assuming full responsibility for any risk, injuries or damage known or unknown which I might incur whilst participating in the Transform8 programme. I knowingly waiver any claim I may have against Transform8 and the instructors for any injuries, illnesses or damages I may sustain as a result of participating in Transform8 and know that I will not receive a refund on any non-participating sessions due to any reason.
